New Patients
At the Skin & Laser Surgery Center we value your time and your privacy. We've provided copies of our new patient information forms for you to download, print, and complete at home. You may bring them into our office during your next visit. They will not be stored or sent online, but are offered here simply for your convenience. Skin & Laser Surgery Center follows Federal HIPAA requirements for maintaining patient privacy and healthcare portability.

Scheduled for Treatment?
Here are some useful reminders:
- Review list of Aspirin and other blood-thinning products and medications and avoid if possible for two weeks prior to surgery, provided your medical doctor has given approval for discontinuing these medications.
- Refrain from consuming alcohol or alcohol containing products for 3 to 4 days prior to surgery.
- Be well rested and eat a good breakfast on the morning of the surgery.
- Wash your hair the night before or morning of your surgery.
- Wear loose fitting, comfortable clothing.
- Take all of your medications as you normally would on the day of the surgery, except the ones listed in our “medications to avoid” hand.
- Plan on spending the day, so bring a good book or a friend to chat. Bring your favorite music CD and we’ll play it.
- Have someone available to drive you home after the surgery, if possible.
- Ask any questions you may have.
